Using Network Manager, first provide the IP settings for your eth0
connection (Wired Network Connection).
Using the same MAC as used in eth0 (this is the mac address of your
NIC) create a new DSL connection using the Network Manager, providing
your username and password.
Connect to the network using this DSL connection.  You may have to
repeat this for every user.

This works for me with my Beetle ADSL2 modem.
